Auto Blocks and Foot Controls are covered by a lifetime warranty when installed as part of a complete dental system. Warranty is limited to repair or replacement by Beaverstate Dental. Products returned to the factory and determined to be defective will be repaired or replaced free of charge. The warranty does not cover light bulbs. Beaverstate Dental reserves the right to void all warrties if any product is installed by other than an authorized technician. Beaverstates warranty does not cover damage to any surface finish, including but not limited to, discoloration or abrasion from cleaners or light sources. BELMED INC.

All BELMED parts listed on the Rogue Dental website will begin with the letters "BM" (example; BM-5000) Definition of a Warranty Return: A product or part covered by the Belmed. Inc. warranty, that fails while the terms of the warranty are in effect. THIS WARRANTY IS GIVEN IN PLACE OF ALL OTHER WARRANTIES, EXPRESS OR IMPLIED, OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE OR OTHERWISE. No statement or claim about the product by any employee, agent, representative or dealer of Belmed Inc. shall constitute a warranty by Belmed Inc. or give rise to any liability or obligation of Belmed Inc. Subject to the next sentence Belmed Inc., warrants that each product or part shall be free from defects in workmanship and materials, under normal use and with appropriate maintenance, for one (1) year form the date of delivery to customer. For plastic, rubber and disposable parts or items Belmed, Inc. warrants only that each such part and item shall be free from defects in workmanship and materials at the time of delivery to the customer. Belmed Inc.’s obligation for breach of this warranty, or for negligence or otherwise, shall be strictly and exclusively limited to Belmed Inc.’s choice of repair or replacement of the product or part. This warranty shall be void for any product on which the serial number has been altered, defaced or removed. Belmed, Inc. shall not be liable for any damage, injury or loss arising out of the use of the product, whether as a result of a defect in the product or otherwise, if prior to such damage, injury or loss, the product was (1) damaged, misused, or misapplied; (2) repaired, altered or modified by persons other than Belmed, Inc. (3) not installed in strict compliance with applicable codes and ordinances; or (4) not installed by Belmed, Inc. or an authorized Belmed, Inc. dealer.

UNDER NO CIRCUMSTANCES SHALL BELMED INC. BE LIABLE FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES AS THOSE TERMS ARE DEFINED IN THE UNIFORM COMMERCIAL CODE.

Equipment Repair - Repair Procedures:

Belmed, Inc. offers evaluation and repair service on all Belmed Products. We provide excellent turnaround on all products with customer approval for service-repair work. Initial

$35.00 inspection fee will be charged.

Shipping Procedures:

Products returned for repair will be accepted without a return authorization number. Specify that it is an item for repair and marked to the attention of the Repair Service Center.

Returning Goods:

  1. The following will Not be accepted for credit:
    • Rubber goods over 30 days old
    • Special order items
    • Rubber goods not in original, sealed package
    • Goods without a Return Goods Authorization (RA)
    • All hoses
    • Merchandise more that 90 days old (from Belmed inc. invoice date.)
  2. A 20% restocking charge will be deducted from the original purchase amount for all credits issued, except for return due to:
  3. Belmed Inc. order entry error
  4. Warranty returns
  5. There is a minimum restocking charge of $10.00
  6. Return Authorization (RA) numbers are valid for 30 days after the date issued.
  7. Freight charges will not be credited.
  8. Freight collect shipments are not accepted.
  9. If merchandise is found to have been misused or in any way damaged after shipment from Belmed Inc., credit will be made less charges for repair and less the 20% restocking charge. If merchandise is not salvageable, no credit will be issued.
  10. All claims for damage or shortage occurring in transit must be made by the customer directly to the carrier.

Warranty Returns:

  1. Merchandise returned for warranty credit or replacement must have been purchased from Belmed within the specified warranty period or proof of installation within that time will be required.
  2. Defective items will, at Belmed Inc.’s discretion, either be repaired or replaced with equivalent merchandise.
  3. If merchandise is found to not be defective the 20% restocking fee will be charged.
  4. Freight charges to return defective merchandise to Belmed Inc. are not reimbursable.
  5. Freight charges to ship replacement or repaired merchandise to customer will be paid by Belmed Inc.

Customer Instructions:

Please follow the procedures outlined below so there is no delay of credit to your account.

You must have a Return Authorization (RA) number before returning any merchandise to Belmed Inc. - except for out-of-warranty repairs for servicing. Credit may be issued only after the merchandise is returned to and inspected by Belmed Inc. and determination is made that credit is due.

To Receive a Return Authorization:

  1. Call your Belmed Inc. Customer Service Representative.
  2. Be prepared to provide the following information:
  3. Our invoice number and date.
  4. Product description, product number, and serial number (if applicable).
  5. Reason you wish to return the merchandise.
  6. Your Customer Service Representative will give you a RA number. A separate RA number will be provided for each Belmed Inc. invoice reference. The RA number authorizes our receiving department to accept the merchandise.
  7. Enclose a copy of our original invoice.
  8. Send the merchandise freight pre-paid.

Shortages:

CHAPMAN-HUFFMAN INC.

All products manufactured or distributed by Chapman-Huffman Inc. are warranted for One Year from the date of purchase (except for New or Rebuilt circuit boards - Three months and Fiber Optic & Power Optic tubing assemblies and Sterilizer gaskets - Six months) under normal and proper use against defective material and workmanship. Any product or part returned to us post paid within hte warranty period, as a result of defective material or workmanship, will be repaired or replaced (at our option) without charge The warranty does not cover damage from improper installation. Chapman-Huffman Inc. liability shall be limited to said repair or replacement and shall not include loss of use, inconvenience or any kind of incidental , consequential or special damages due to failure of this product.

All DCI parts listed on the Rogue Dental website will begin with the letters "DC" (example; DC-0070) DCI has put significant time and effort into designing, manufacturing or selecting all of their products in their catalog. Therefore they are able to fully guarantee every product they sell to be free of manufacturing defects free from the time of purchase (except Fiber Optic tubings Handpiece turbines and Copeland Compressor Heads which are guaranteed for 6 months). Should any product fail, for whatever reason due manufacturing defect, DCI will repair or replace the product at no charge to the customer. FOREST DENTAL

Forest warrants its products to be free from defects in material and workmanship only. No other warranties are expressed or implied. This warranty shall extend for five years unless otherwise stated. Written notice of this warranty is limited to repair parts or replacement items by FOREST. This warranty is void if items are carelessly used or improperly maintained or installed. This warranty does not cover damage that results from using cleaning, disinfecting or sterilization chemicals and processes. This warranty does not cover light bulbs. Handpiece illumination and heated syringe tubings are warranted for six months to be free from defects in material and workmanship only. Factory repairs are covered for a period of 90 days. All Parts Warehouse parts listed on the Rogue Dental website will begin with the letters "PW" (example; PW-C1802W) Parts Warehouse is dedicated to manufacturing and supplying quality parts combined with excellent support and service. If you feel that a product did not perform at a satisfactory level for its intended use, please contact Rogue Dental at 913-583-3322 for assistance. Parts Warehouse products are guaranteed for 2 years This warranty excludes normal expected service items such as but not limited to: filters, o-rings, vacuum hoses, gaskets, scaler tips, water valve/regulator components, syringe o-rings/tips, HVE, SE, or other vacuum valve user serviceable o-rings or seals. Items that require regular maintenance may not be covered if basic consumer servicing is lacking. All returned parts must have been properly installed and used as intended. Bulbs are warranted to be free from manufacturing defects only at time of shipment; useful life of bulbs is not warranted. Returns of used vacuum equipment/valves cannot be accepted.( Subject to limitations listed in return policy)

PARTS WAREHOUSE RETURN POLICY

Parts Warehouse may not warrant, at our discretion, returns/exchanges due to the following:

(a) Products considered to be of a consumable nature

(b) Changes in color caused by natural or artificial light

(c) Products which are not properly installed, used, or cleaned and maintained

(d) Defects, damage, or other conditions caused, in whole or in part, by misuse, abuse, negligence, alteration, accident, freight damage, tampering

(e) Non-stocked items or special orders

(f) Connection to an improper voltage supply

(g) Shipping damage must be immediately brought to the attention of the carrier. Retain all original packaging and contact UPS at 1-800-PICK-UPS or have your USPS postal clerk verify damage. Claims for shipping damage must be filed with the carrier.

(h) Products that have received damage due to inadequate return packaging. Parts Warehouse shall not be liable for:

(a) Costs and expenses of routine maintenance and cleaning

(b) Failure to seek and obtain repair or replacement in a timely manner

(c) Charges by anyone for adjustments, repairs, replacement parts, installation, or other work performed upon or in connection with such products which are not expressly authorized in writing in advance by Parts Warehouse.

(d) Representations and warranties made by any person or entity other than Parts Warehouse.

PARTS WAREHOUSE’S only obligation under our warranty is the repair or replacement of defective parts. Parts Warehouse shall not be liable for and hear by disclaims any direct special, indirect, incidental, exemplary or consequential DAMAGES OR DELAYS INCLUDING, BUT NOT LIMITED to, damages for loss of profits or income, loss of use, downtime, cover, and employee or independent contractor wages, payments and benefits Items will not be accepted without a RETURN AUTHORIZATION NUMBER.

The RPI parts must meet with your complete satisfaction. All RPI parts are manufactured to meet or exceed original equipment manufacturer performance requirements.

Should any RPI part not meet with your complete satisfaction, please contact ROGUE DENTAL at 913-583-3322 as soon as possible to obtain a Returned Goods Authorization

(RGA) number prior to returning any part. We will gladly arrange an exchange or credit. (No parts may be returned without an RGA number and returned parts may be subject

to a 15% restocking fee.
